Specialized care for pelvic venous disease and fibroid-related pelvic pain
Living with chronic pelvic pain can be frustrating, especially when symptoms persist without clear answers. At California Pelvic Pain Institute, we understand how deeply pelvic pain can affect daily life, comfort, and overall well-being. Our goal is to help patients feel heard, supported, and hopeful as we work together to identify the underlying cause of their symptoms.
Because pelvic pain can have multiple contributing factors, we take a collaborative, multidisciplinary approach to care. Our team works closely with OB-GYN physicians, pelvic floor therapists, and other specialists to ensure that each patient receives a comprehensive evaluation and treatment plan tailored to their unique needs.
By combining advanced imaging, minimally invasive treatments, and coordinated care, California Pelvic Pain Institute strives to help patients move toward relief, improved comfort, and a better quality of life.
Conditions We Treat
Our team focuses on conditions that commonly contribute to chronic pelvic pain and pelvic vascular issues. By identifying the underlying cause of symptoms, we can develop treatment plans designed to improve comfort and long-term pelvic health.
Pelvic Venous Disease
Pelvic venous disease, sometimes referred to as pelvic congestion syndrome, occurs when pelvic veins become enlarged or weakened, leading to chronic pelvic pain, pressure, or discomfort. Minimally invasive procedures such as pelvic vein embolization may help reduce symptoms by addressing the affected veins.
Fibroid Disease
Uterine fibroids are non-cancerous growths that develop in the uterus and may cause symptoms such as pelvic pressure, heavy menstrual bleeding, or abdominal discomfort. Uterine fibroid embolization (UFE) is a minimally invasive treatment that targets the blood supply feeding fibroids, helping reduce their size and associated symptoms.
Minimally Invasive Treatments
Many pelvic vascular conditions can be treated through image-guided embolization procedures that require only a small catheter insertion rather than large surgical incisions. These treatments are designed to target specific blood vessels while preserving surrounding healthy tissue.
Hearing from others who have experienced similar symptoms can help patients feel more confident about seeking care. Many individuals who visit California Pelvic Pain Institute have spent years searching for answers before discovering the underlying cause of their pelvic pain.

I am writing this review on behalf of my father, who has been a patient of Dr. Derrick Tran at PEDES Orange County. From the very first visit, we knew we were in great hands. Dr. Tran is an exceptionally knowledgeable, compassionate, and dedicated physician who truly goes above and beyond for his patients. He takes the time to thoroughly explain every detail, ensuring we understand all options and feel confident in the care being provided. No question ever goes unanswered, and his patience and expertise are unmatched.
Beyond Dr. Tran, the entire staff at PEDES Orange County is absolutely amazing! From the front desk team to the medical assistants, nurses and techs, everyone is kind, professional, and incredibly accommodating. They make scheduling easy, treat every patient with the utmost respect, and ensure a smooth experience at every visit.
My father and I are beyond grateful for the outstanding care he has received.

Yesterday I had a stent procedure with Dr. Goldstein.
I wanted to thank the entire team.
Dr. Hewitt saw me first for a consultation. He knew from my symptoms that I needed some help. He ordered an MRV and helped me get the right procedures ordered. I felt like a priority from the time I met him.
I felt heard.
Yolanda the scheduler was very professional and prompt. Helping me get scheduled quickly.
Juliana, Mariana, Goli, Ghazal and Chelsea took care of me while I was there.
I was nervous for the procedure and feeling vulnerable. I also have an allergic reaction when I get exposed to cold temperatures.
It’s called Cold Urticaria.
Being in an operating room can be painful due to the cold temperatures. Mariana and Goli went above and beyond to make sure to I felt safe, covered and super warm. They used blankets, and even multiple heating packs.
After the procedure Juliana stayed with me and made sure I was feeling okay and warm. I was her last patient, but she didn’t make me feel rushed or like an inconvenience.
I just wanted everyone to know what a great experience I had.
The doctors were both intelligent and caring…highly recommend them!
I have to go back for another procedure and I’m glad it will be with the incredible Dr. Goldstein and his awesome team.

We are incredibly grateful to the dedicated team at [Pedes Orange County] and to Dr. Hewett and Dr. Goldstein for their ongoing care for my father. He has faced multiple vascular surgeries this year, and his recent ultrasound showed concerning findings. We were hopeful for one more procedure before year’s end to help prevent further complications.
Unfortunately, we’ve been unable to reach the team despite our best efforts, and my father’s foot condition continues to worsen. This is an urgent situation for him and our family, and we’re seeking immediate guidance and a clear plan of care.
If anyone has recommendations for:
how best to reach the care team (phone, after-hours lines, or patient portal),
alternative contact points within the same medical group or hospital,
or guidance on next steps for urgent vascular/foot care, we would be profoundly grateful.

I had such a great experience with this vascular surgery office. The doctors and the entire staff are very professional, kind, and genuinely nice people. From the beginning, everything was well organized and clearly explained, which made the whole process much less stressful.
I’m really glad I chose to have my surgery here. I had been to other facilities before, so I truly can compare - and the level of care here was noticeably better. They take their work seriously, are very knowledgeable and responsible, and they did thorough ultrasounds and follow-ups to make sure everything was done properly and working as it should.

I saw Dr. Hewett today for my initial consultation. He was kind, professional, and explained everything clearly. I felt confident in his assessment so far. While it’s hard to ease my fears given the seriousness of my medical concerns, Dr. Hewett was patient, gave me plenty of time, and didn’t rush the visit. The office was spotless, and the front desk staff and the doctor’s assistant were friendly and very competent. I also truly appreciate being seen on such short notice. Thank you!
Minimally Invasive Treatment vs Traditional Surgery
Many pelvic vascular conditions can be treated using minimally invasive procedures that focus on precision and faster recovery.
Minimally Invasive Treatment
Outpatient Procedure
Patients typically return home the same day.
Light Sedation
Comfortable twilight sedation is commonly used.
Shorter Recovery Time
Many patients resume normal daily activities sooner.
Fewer Post-Procedure Restrictions
Recovery often involves fewer limitations compared to surgery.
VS
Minimally Invasive Treatment
Hospital Stay
May require an overnight or multi-day hospital stay.
General Anesthesia
Performed under full anesthesia.
Longer Recovery Period
Recovery may take several weeks before normal activities resume.
Post-Surgical Recovery Care
Follow-up care and a longer healing period may be required.

Frequently Asked Questions
What causes chronic pelvic pain?
Chronic pelvic pain can have several potential causes. In many cases, symptoms may be related to pelvic venous disease, pelvic congestion syndrome, uterine fibroids, or other vascular conditions within the pelvis. A thorough medical evaluation and imaging studies can help determine the underlying cause.
What is pelvic venous disease?
Pelvic venous disease occurs when pelvic veins become enlarged or weakened, which may lead to symptoms such as pelvic pressure, aching pain, or discomfort that worsens throughout the day. Treatment options may include pelvic vein embolization, a minimally invasive procedure designed to address the affected veins.
What are uterine fibroids?
Uterine fibroids are non-cancerous growths that develop within the uterus. While many fibroids do not cause symptoms, others may lead to heavy menstrual bleeding, pelvic pressure, or abdominal discomfort.
What is uterine fibroid embolization (UFE)?
Uterine fibroid embolization is a minimally invasive treatment that reduces blood flow to fibroids by targeting the small branches of the uterine artery that feed them. This targeted approach helps shrink fibroids while preserving blood flow to healthy uterine tissue.
Are minimally invasive treatments safe?
Minimally invasive procedures have been widely used for many years and are designed to treat specific conditions with smaller incisions and shorter recovery times than traditional surgery. Your physician will review the benefits and potential risks during your consultation to help determine whether treatment may be appropriate for your situation.
When should I see a specialist for pelvic pain?
If pelvic pain persists for several months, worsens over time, or interferes with daily activities, it may be helpful to consult a specialist who can evaluate possible vascular or structural causes of your symptoms.
